The physicians and staff of Advanced Aesthetics are pleased that you have chosen us. Our facility, with state-of-the-art equipment, allows our well-trained physicians and personnel to provide the most effective treatment for healthy and younger-looking skin. We look forward to helping you care for and improve your skin.
Below you will find your new client forms. Please complete all sheets and bring them with you to your appointment. If you have any questions or cannot keep your appointment, please call our scheduling staff at (888) 337-6722 extension 6. If you need to reschedule your appointment, please call at least 24 hours in advance.
- Patients under the age of 18 years old must be accompanied by a parent or legal guardian.
- Legal guardians must bring written proof of guardianship.
- Complete all new patient forms and bring them to your first appointment
- Bring all your medications to your appointments to keep records up-to-date
- Have your current insurance card(s) with you
